All Athletes need to complete these athletic forms prior to participation:

1. Physical/Alternate year form.  (Physicals needed 6th, 9th, and 11th  grade)
2. WIAA Parent-Athlete Rules of Eligibility Sign-Off Form
3. Athletic Emergency Locator Form
4. Extracurricular Code Handbook Student/Parent Pledge Sheet
5. Concussion Parent/Athlete Agreement Form

Athletic fee must also be paid prior to participation.
